Clinical Observation of Stomach Cancer in Korea

Abstract
During last 10 years the incidence of stomach cancer among the Koreans has been increased than
that of occidentals. According to the series of reports from well known clinics in Korea, the number
of stomach cancer patients is seemed to be increasing every year. With this fact, it was decided to
have statistical analysis of stomach cancer in our hospital regarding its general clinical behavior along
with the results of surgical treatment.
The materials used in this study were 1132 patients who were admitted in Severance Hospital
during last 14 and half years from Jan. 1, 1955 to June 30, 1969.
We devided in two periods above 14 and half years that are the earlier, from Jan. 1, 1955 to
Dec. 31, 1962 and the latter from Jan. 1, 1963 to June 30, 1969 and compaired two stages each
other in clinical aspect.
Results may be summarized as follows.
1) During the last 14 and half years, the percentage ratio of stomach cancer to the total number
of in-patient was 1.23% and there was no difference in earlier and latter period.
2) In the latter period, peak incidence was in the age group of 50 to 59 and its number of patient
was 271 (37.1%). Comparing with earlier period, that is more elder group.
3) Male to female ratio was 3:1 in earlier and 2:1 in the latter.
4) Subjective complaints in order of frequency were as follows: in the earlier period upper adb
ominal pains 210 cases (92.1%), indigestion 203 cases (89.0%) and anorexia 80 cases (35.0%) and
in the latter period, epigastric discomfort or pain 581 cases (79.5%), indigestion 462 cases (63.2%),
vomitting 240 cases (32.8%) and general weakness 220 cases (30.1%).
5) Objective findings in order of frequency were as follows: in the earlier period, palpable mass
112 cases (49.1%), weight loss 86 cases (37.7%) and vomotting 80 cases (35.0%) and in the latter
period, above signs were similar as earlier period. But tenderness, bloody stool, ascites, jaundice and
melena were more increased in somewhat than earlier period.
6) The operability was similar in 2 periods, but resectability was much higher in the latter period,
although the curatability was decreased in the latter period.
These might be said our surgical group have been carried out more aggressive approaches in
management of stomach cancer in the latter period and contararily speaking we have patients in
progressed stages, therfore, early detection is urgent to improbve the results of surgical treatment.
7) The localization of tumor was similar in 2 periods.
8) The most common post-operative complication was found infection in all periods, but the
complication rate was much lessened in the latter period and operative mortality was also lessened
in the latter period.
